What It Is
Amla, also known as Indian gooseberry, is the fruit of Phyllanthus emblica L. (syn. Emblica officinalis Gaertn.). It contains hydrolyzable tannins, polyphenols, organic acids, flavonoids, and vitamin C. Standardized amla fruit extracts have been investigated primarily for their effects on lipid metabolism, vascular function, glucose regulation, and inflammatory biomarkers.

Research suggests standardized amla fruit extract may help maintain healthy cholesterol, triglyceride, glucose, and vascular-function markers already within normal ranges.
The most developed area of human research involves cardiometabolic biomarkers, particularly LDL cholesterol, triglycerides, and high-sensitivity C-reactive protein. However, the available trials are generally small and short, and results vary across formulations and study populations.
What to Expect
Most clinical studies have evaluated standardized amla fruit extracts at total daily intakes of approximately 500–1,000 mg for 8–12 weeks. Several studies have reported improvements in lipid and inflammatory biomarkers during this period. These effects are generally identified through laboratory testing rather than through noticeable day-to-day changes.
Mild digestive effects have been reported in some studies, including:
Dyspepsia
Abdominal discomfort
Nausea
Changes in bowel habits
These effects appear uncommon and have generally been mild. Taking amla with food may improve tolerability for individuals with sensitive digestion.
Medication Caution
Amla may influence blood glucose, blood pressure, and platelet function. Individuals taking glucose-lowering medications, antihypertensive medications, anticoagulants, or antiplatelet medications should consult a qualified healthcare professional before use.

What Is Amla?
Amla is the edible fruit of the Phyllanthus emblica tree, which is native to India and other parts of South and Southeast Asia. It is also known as:
Indian gooseberry
Amalaki

The fruit has been used as both a food and a traditional botanical preparation. Although several parts of the plant have been used traditionally, most modern human research has evaluated the fruit or a concentrated fruit extract.
Amla contains several classes of naturally occurring compounds, including:

The concentration of these constituents varies substantially between fresh fruit, dried powder, juice, and proprietary extracts. A standardized tannin extract should not automatically be assumed to provide a clinically meaningful amount of vitamin C unless vitamin C is separately quantified on the label.
How Amla Works

Laboratory and animal studies suggest that amla constituents may influence hepatic HMG-CoA reductase activity, cholesterol transport, antioxidant defenses, and endothelial nitric-oxide availability.
These proposed mechanisms help explain why amla has been investigated for lipid, glucose, inflammatory, and vascular-function outcomes. However, many mechanistic findings have not been fully confirmed in humans and should not be interpreted as evidence that amla prevents or treats cardiovascular or metabolic disease.
Absorption and Metabolism
The hydrolyzable tannins found in amla can be broken down within the gastrointestinal tract into smaller phenolic compounds, including gallic-acid- and ellagic-acid-related metabolites. Some constituents may also undergo further transformation by intestinal microorganisms.

What the Research Shows
While individual studies vary, the most consistent areas of human research involve lipid markers, inflammatory biomarkers, glucose regulation, and surrogate measures of vascular function.

Research Summary: Meta-analyses of small randomized trials suggest that amla may reduce LDL cholesterol by approximately 15–25 mg/dL under study conditions, with possible improvements in triglycerides, fasting glucose, and inflammatory markers. However, findings vary considerably across studies and extract preparations, and the overall certainty of evidence is low because of small sample sizes, short treatment periods, and substantial heterogeneity. Evidence for HDL cholesterol and HbA1c improvement is less consistent.

Research Summary: In the nine-trial cardiovascular meta-analysis, amla was associated with an average reduction in high-sensitivity C-reactive protein of approximately 1.7 mg/L compared with placebo. Smaller formulation-specific trials have reported improvements in surrogate measures of endothelial function, particularly with 500 mg twice daily.

Capros® is standardized to a minimum of 60% low-molecular-weight hydrolyzable tannins and has been evaluated in several human trials. Another standardized extracts containing 35% polyphenols, 8% triterpenoids, and 10% amla oil has also been shown to produce favorable lipid findings in clinical studies.

In clinical research settings, Capros®, a standardized aqueous amla fruit extract has most commonly been evaluated at the following dosing ranges:

The 1,000 mg daily dose has been used most consistently in studies evaluating lipid, endothelial-function, and inflammatory outcomes.

These dosing ranges reflect clinical study protocols. Because the chemical composition of amla products can vary substantially, doses of Capros®, other proprietary extracts, and whole-fruit powder are not directly interchangeable.
Safety and Tolerability
Amla fruit extracts have generally been well tolerated in short-term clinical studies.
Reported side effects have included:
Dyspepsia
Mild gastritis
Abdominal discomfort
Nausea
Headache
Changes in bowel habits
Amla may have additive effects when combined with:
Glucose-lowering medications
Antihypertensive medications
Antiplatelet medications
Other supplements that influence platelet function or glucose regulation
These effects are most likely when starting higher doses and often improve when taken with meals.
Individuals taking prescription medications should consult a healthcare professional before use.
Safety Considerations
Avoid use during pregnancy or breastfeeding unless specifically recommended by a qualified healthcare professional. Adequate safety data are not available.
Use additional caution in individuals with:
A history of hypoglycemia
A bleeding disorder
Concurrent anticoagulant or antiplatelet therapy
Multiple antihypertensive medications
Planned surgery or an invasive procedure
Advanced liver disease
Advanced kidney disease
There is no established amla dose adjustment for reduced kidney or liver function because these populations have not been adequately studied.
Because amla may reduce laboratory platelet aggregation, discontinuation before elective surgery may be considered in consultation with the treating clinician. The appropriate interval has not been established in clinical trials.
Safety and efficacy have not been adequately established in children or adolescents.

Amla has been evaluated in multiple randomized trials and several systematic reviews and meta-analyses.
When reviewing the evidence, several limitations should be considered.

The current evidence supports a possible role for standardized amla extract in maintaining healthy cardiometabolic biomarkers. It does not establish prevention of cardiovascular events, diabetes, or other chronic diseases, and it does not support substituting amla for evidence-based pharmacologic treatment when medication is clinically indicated.
Summary
Amla has a developing evidence base for cardiometabolic support, with small clinical trials suggesting possible improvements in LDL cholesterol, triglycerides, fasting glucose, inflammation, and vascular function. However, the evidence remains limited by short study durations, small sample sizes, varied formulations, and a lack of cardiovascular-outcome data. Capros® has the clearest formulation-specific research, typically at 500–1,000 mg daily. Amla is best considered a complementary option alongside established lifestyle and medical care.
